Our complete guide to vertigo care in Sarasota covers the causes we see, the imaging we use, and what to expect at an evaluation.

Doing the Epley maneuver for vertigo can be a Band-Aid to actually correcting the cause. This can be a debilitating condition, leaving those who suffer from it feeling off-balance, dizzy, and disconnected from their daily lives. While the Epley Maneuver is often the go-to solution for positional vertigo, it doesn’t work for everyone and may not address the underlying causes. At Lavender Family Chiropractic in Sarasota, we offer a better alternative that focuses on restoring balance and improving overall well-being through upper cervical chiropractic care.

Understanding Vertigo and Its Common Treatments (Usually the Epley maneuver for vertigo)

Vertigo is often caused by disturbances in the inner ear, such as dislodged calcium crystals within the semicircular canals. This is a hallmark of benign paroxysmal positional vertigo (BPPV), one of the most common types of vertigo. The Epley Maneuver, a series of head and body movements, aims to reposition these crystals to alleviate symptoms. While effective for some, it can be uncomfortable, fail to provide long-term relief, and doesn’t address deeper neurological or structural causes of vertigo.

Other types of vertigo, such as those stemming from vestibular migraines, Meniere’s disease, or head and neck trauma, may not respond to the Epley Maneuver at all. This is where a holistic approach like upper cervical chiropractic care becomes invaluable.

Why Choose Upper Cervical Chiropractic Care in Sarasota?

At Lavender Family Chiropractic, located conveniently in Sarasota, we focus on the alignment of the upper cervical spine—specifically the atlas (C1) and axis (C2) vertebrae. These vertebrae play a crucial role in housing and protecting the brainstem, which is a key player in maintaining balance and coordinating signals between the brain and body. Even subtle misalignments in this area can disrupt normal nerve function, leading to symptoms.

Key Benefits of Upper Cervical Chiropractic for Vertigo:

  1. Targeting the Root Cause: Unlike the Epley Maneuver, which primarily addresses symptoms, upper cervical chiropractic care focuses on correcting spinal misalignments that may be contributing to vertigo.
  2. Non-Invasive and Gentle: Upper cervical adjustments use precise, gentle techniques that avoid forceful manipulation. This makes the process comfortable and safe for patients of all ages.
  3. Improved Neurological Function: By restoring proper alignment, we help the nervous system function optimally, reducing dizziness and other vertigo-related symptoms.
  4. Holistic Health Benefits: In addition to relieving vertigo, proper spinal alignment can improve overall well-being, including better posture, reduced headaches, and enhanced energy levels.

What the Research Says About the Epley Maneuver and the Neck

For posterior-canal BPPV, the Epley maneuver is not something to work around; it is the evidence-based first step. A Cochrane review found Epley effective compared with sham for clearing the symptoms of posterior-canal BPPV (Hilton & Pinder, Cochrane 2014), and the AAO-HNS guideline recommends Dix-Hallpike testing to confirm the diagnosis followed by canalith repositioning (Bhattacharyya et al., Otolaryngol Head Neck Surg 2017). We perform or refer for it whenever positional testing is positive. The honest limitation is different: BPPV can recur (Kim & Zee, N Engl J Med 2014), and when positional testing is negative, Epley has nothing to act on. That is where a different question becomes worth asking.

The different question is whether the neck is involved. Cervicogenic dizziness has no gold-standard test and is diagnosed by excluding other causes and by its relationship to neck pain and neck movement (Reiley et al., Arch Physiother 2017), so a person whose Epley did not help, or whose vertigo keeps returning, may be carrying a second problem the maneuver was never meant to address. A randomized trial of two upper cervical manual techniques reduced dizziness compared with placebo, with the gain still present at 12 months (Reid et al., Phys Ther 2014). We do not replace Epley; we add 3D imaging of the atlas, a correction matched to those images, and follow-up measurement for the neck component.

Frequently Asked Questions

Should I try the Epley maneuver before seeing an upper cervical chiropractor?

If a physician or ENT has confirmed benign paroxysmal positional vertigo, the Epley maneuver is the right first step because it repositions the crystals in the inner ear. Many people get relief from it. The question we help answer is what to do when vertigo keeps coming back or the Epley does not seem to apply. That is when we image the atlas with cone-beam CT to see whether the upper neck is contributing.

Why does my vertigo return after the Epley maneuver works?

Recurrence can happen for several reasons, and one that is often overlooked is the upper neck. The brainstem combines inner-ear signals with position input from the atlas region. If the atlas is misaligned, the neck keeps feeding a distorted signal even after the ear is repositioned. Correcting the atlas addresses that input. We measure with thermography before and after each correction, so we know whether the nervous system is actually settling rather than assuming it.

How is upper cervical care different from the Epley maneuver?

The Epley works on the inner ear. Upper cervical care works on the alignment of the first two bones in the neck, which sit around the brainstem. The Knee Chest correction is precise and low-force, calculated from 3D cone-beam CT, with no twisting or rotation of the neck. You rest afterward. The two approaches address different parts of the balance system and can be used together when both are needed.

Can an upper cervical chiropractor tell me whether I have BPPV?

We can recognize the pattern, but BPPV is confirmed by a physician or ENT through positional testing, and serious causes of vertigo should always be ruled out medically first. What we add is the piece most workups skip: an actual image of the atlas and axis and a thermography scan showing how the nervous system is regulating. If the upper neck is not involved, we say so and refer you back.
