In short: There is no board-certified “fibromyalgia specialist.” In Sarasota, fibromyalgia is diagnosed and managed by rheumatologists, primary care physicians, and pain management physicians, with sleep medicine and physical therapy alongside, and evaluated at the top of the neck by an upper cervical chiropractor, because the condition is now understood as a disorder of central pain processing and the upper neck is the largest single input into that processing. Florida chiropractors do not hold specialty designations, and we do not describe ourselves that way. This page explains who does what, how to tell which provider you need first, and where Lavender Family Chiropractic fits. Call (941) 243-3729.
Why “fibromyalgia specialist” is a confusing search
Fibromyalgia used to be a rheumatologist’s diagnosis of exclusion. It is now classified as a chronic primary pain condition. A 2026 review in the Journal of Pain Research describes it as a multifactorial syndrome of widespread pain, fatigue, sleep disturbance, and cognitive dysfunction, diagnosed clinically by American College of Rheumatology criteria, with peripheral and central mechanisms both involved. A 2026 meta-analysis in Anesthesia and Analgesia covering 30 million people across 38 countries put the general-population prevalence at about 1.4 percent. Because no single organ is diseased, no single specialty owns it, and patients in Sarasota routinely see three or four providers before anyone coordinates the picture.
Who treats fibromyalgia in Sarasota
Rheumatologist. Confirms the diagnosis against the ACR criteria and, more importantly, rules out the conditions that mimic it: inflammatory arthritis, lupus, polymyalgia, thyroid disease. Once those are excluded, many rheumatologists hand management back to primary care.
Primary care physician. Usually where long-term management lives: medication, sleep, exercise prescription, and referrals.
Pain management physician. For medication strategy, trigger point injections, and coordinated multidisciplinary programs.
Sleep medicine. Non-restorative sleep is central to fibromyalgia, and untreated sleep apnea makes it worse. A sleep study is worth asking about if it has never been done.
Physical therapist. For graded exercise, which has the strongest evidence of any single intervention. A Cochrane review of 13 randomized trials found aerobic exercise improved health-related quality of life and was well tolerated, which is why graded exercise is the backbone every other provider on this list builds around.
Upper cervical chiropractor. For the top of the neck. Fibromyalgia is a disorder of how the central nervous system processes pain, and the brainstem, where that processing is set, sits inside the atlas (C1). Many of our Sarasota fibromyalgia patients date their symptoms to a car accident, fall, or whiplash, and most have neck pain and headaches as part of the picture. That is not a coincidence of our practice. A controlled study in Arthritis and Rheumatism followed 102 patients with neck injury and 59 with leg fractures, none of whom had a chronic pain syndrome before the trauma, and found fibromyalgia developed in 21.6 percent of the neck-injury group against 1.7 percent of the leg-fracture group, a thirteenfold difference tied specifically to injuring the neck. A review in Neuroscience summarizes the substantial evidence for altered central nervous system pain processing in fibromyalgia alongside an emerging peripheral component. We measure the alignment of the atlas with 3D imaging and correct it without twisting, which matters in a patient whose tissues are already sensitized. Our guide to fibromyalgia and small-fiber neuropathy covers the nerve findings that overlap.
How the options compare
| Provider | What they evaluate | When they come first |
|---|---|---|
| Rheumatologist | ACR criteria; exclusion of inflammatory and autoimmune mimics | First diagnosis; any joint swelling, rash, or abnormal blood work |
| Primary care physician | Long-term medication, sleep, exercise, coordination | Ongoing management |
| Pain management | Medication strategy, injections, programs | Pain not controlled by first-line care |
| Sleep medicine | Sleep study, apnea | Non-restorative sleep, snoring, daytime exhaustion |
| Physical therapist | Graded exercise, pacing | Deconditioning, flare cycles |
| Upper cervical chiropractor (Lavender Family Chiropractic, Sarasota) | Alignment of the atlas and axis with 3D CBCT; autonomic regulation along the spine | Fibromyalgia with neck pain, headaches, or dizziness; onset after a car accident or whiplash; brain fog and sleep disturbance; care that has plateaued; alongside the above |
How to tell which one you need first
Rheumatologist first if you have never been formally diagnosed, or if you have joint swelling, a rash, fevers, or abnormal inflammatory markers; those need to be excluded before the fibromyalgia label is accepted.
Sleep medicine if you snore, wake unrefreshed every day, or have never had a sleep study.
Add an upper cervical evaluation if your fibromyalgia began after a car accident, fall, or whiplash; if neck pain, headaches at the base of the skull, dizziness, or jaw pain are part of your pattern; if brain fog and poor sleep dominate; or if medication and exercise have plateaued. Our guides to fibro fog and fibromyalgia flares and triggers cover the two patterns we see most.
What an upper cervical fibromyalgia evaluation at Lavender Family Chiropractic involves
We start with your rheumatology and lab reports; the exclusion of inflammatory disease is the foundation. Then a history that goes back to the first symptom and every neck injury before it, a gentle examination of the upper neck and nervous system, paraspinal infrared thermography, which reads side-to-side autonomic regulation without touching the skin, and, when the examination points to the upper neck, 3D CBCT imaging of the atlas and axis. If the upper neck is involved, a low-force Knee Chest correction with no twisting, cracking, or rotation, delivered only when thermography shows the correction is needed, which keeps visits from over-stimulating a sensitized system. Results vary by person; we measure before and after. If the neck is not involved, we say so.

Common questions about finding a fibromyalgia specialist in Sarasota
Which specialist treats fibromyalgia in Sarasota? A rheumatologist for diagnosis and exclusion of mimics; your primary care physician for long-term management; pain management and sleep medicine as needed; a physical therapist for graded exercise; and an upper cervical chiropractor for the top of the neck, which feeds the central pain processing fibromyalgia disturbs.
Is a chiropractor a fibromyalgia specialist? No. Florida chiropractors do not hold specialty designations, and we do not describe ourselves that way. We evaluate and correct the atlas and axis; the diagnosis belongs to your physician.
Is upper cervical care gentle enough for fibromyalgia? The correction is low force with no twisting or cracking, and it is made only when thermography shows it is needed. Most patients describe a light pressure at the side of the upper neck and are surprised it is already over.
Why would the neck matter in a whole-body condition? Because fibromyalgia is a processing disorder, and the brainstem that does the processing sits inside the atlas. Neck injury is a common starting point in the histories we take.
Should I stop my medication? No. Keep your physician and your plan. We work alongside them and send our findings back.
How quickly can I be seen? We hold time for same-day and next-day new patient visits. See our same-day chiropractor in Sarasota page or call.
