I want to be clear from the very beginning about something important: this is an education-first article. Small-fiber neuropathy is, in the overwhelming majority of cases, a medical condition with systemic and metabolic roots. It usually needs a proper medical workup and medical management, not a treatment for your neck. What Is Small-Fiber Neuropathy?

Your peripheral nervous system is made up of nerve fibers of different sizes. The “large” fibers are the thick, heavily insulated nerves that carry signals for muscle strength, vibration sense, position sense (knowing where your foot is without looking), and the reflexes a doctor checks by tapping your knee. The “small” fibers are the thin nerve fibers, some lightly insulated and some with no insulation at all, that carry a very different set of signals.

Small fibers do two main jobs:

  • Sensory work. They carry sensations of pain, temperature (hot and cold), and certain kinds of touch. When they misfire, you feel burning, tingling, prickling, itching, or pain that seems out of proportion to any injury.
  • Autonomic work. Many small fibers are part of the autonomic nervous system, the “automatic” network that quietly regulates sweating, blood pressure control, heart rate, digestion, and blood flow to the skin. When these fibers are damaged, people can experience problems like abnormal sweating, lightheadedness on standing, dry eyes and mouth, or digestive changes.

Small-fiber neuropathy is a condition in which these thin nerve fibers become damaged or degenerate. Because the small fibers are the ones affected, the classic symptoms are sensory and autonomic, while muscle strength usually stays intact, at least early on. That single fact explains a great deal of the confusion patients experience, and we will come back to it.

What Does Small-Fiber Neuropathy Feel Like?

No two people describe SFN in exactly the same words, but there are patterns that come up again and again. Understanding them can help you put language to what you have been feeling.

Common Sensory Symptoms

  • Burning pain, often in the feet, sometimes described as feeling like the skin is on fire or like walking on hot sand.
  • Tingling and prickling, the “pins and needles” feeling, that may come and go or stay constant.
  • Electric-shock or stabbing sensations that shoot through the feet or hands without warning.
  • Numbness, though people often describe it as a strange “wooden” or “thick” feeling rather than complete loss of sensation.
  • Allodynia, where things that should not hurt do hurt. Bedsheets resting on the feet at night, socks, or a light touch can feel painful.
  • Itching or crawling sensations on the skin with nothing visibly there.

The Classic Pattern

Small-fiber symptoms very often begin in the feet and toes, the parts of the body farthest from the spinal cord, and then slowly work their way upward over months or years. This is called a “length-dependent” pattern, because the longest nerves are affected first. In some people, the hands become involved once the symptoms in the legs reach roughly knee level, producing what clinicians sometimes call a “stocking-and-glove” distribution.

Not everyone follows this map. Some people have patchy, non-length-dependent SFN, where symptoms show up in the face, trunk, arms, or in scattered areas that do not follow the usual bottom-up progression. This pattern is more often linked to certain autoimmune or inflammatory causes and can be easy to dismiss precisely because it does not look “typical.”

Symptoms Are Often Worse at Night

A hallmark of small-fiber pain is that it tends to intensify in the evening and at night. Many people find that the burning or tingling that was a background hum during the busy day becomes loud and hard to ignore when they finally lie down and the distractions fall away. Sleep disruption is one of the most wearing parts of living with SFN.

Autonomic Symptoms

Because small fibers also run the autonomic system, some people notice symptoms that they never would have connected to a nerve problem:

  • Lightheadedness or dizziness when standing up quickly
  • Changes in sweating, either too much or too little
  • Dry eyes and dry mouth
  • Digestive changes such as bloating, early fullness, constipation, or diarrhea
  • Changes in skin color or temperature in the hands and feet
  • Bladder or sexual function changes

When these autonomic features are prominent, it is an important signal to your medical team, because it can point toward specific underlying causes and it deserves careful evaluation.

Why Standard Nerve Tests Often Miss It

Here is the part that catches so many patients off guard. You go to a neurologist or your primary doctor, they order the standard nerve studies, the results come back normal, and you are left wondering whether anything is actually wrong.

The two standard tests are nerve conduction studies (NCS) and electromyography (EMG). Together they are often just called “the EMG.” These tests are genuinely valuable, but it helps to understand exactly what they measure.

Nerve conduction studies work by delivering a small electrical pulse to a nerve and measuring how fast and how strongly the signal travels. Here is the catch: this test primarily reads the large, well-insulated nerve fibers, because those are the fast, strong signal carriers the equipment is designed to detect. EMG, meanwhile, measures the electrical activity of muscles, which reflects the health of large motor nerves.

Do you see the problem? Standard NCS and EMG are essentially blind to the small fibers. In pure small-fiber neuropathy, the large fibers are still healthy, so the large-fiber test comes back normal, even while the small fibers are quietly degenerating and causing very real pain. A normal EMG does not rule out small-fiber neuropathy. It simply tells you that the large fibers are, for the moment, working normally.

This is one of the most important messages I can share with you: a normal EMG is not proof that nothing is wrong. It is a normal result on a test that was never designed to see the fibers responsible for your symptoms. If your burning and tingling have been dismissed on the basis of a normal EMG alone, it may be worth a respectful conversation with your physician about small-fiber-specific evaluation.

How Small-Fiber Neuropathy Is Properly Diagnosed

Because the standard tests miss it, diagnosing SFN requires a different approach. Good diagnosis rests on three legs: a careful clinical history and exam, specialized testing that actually looks at small-fiber function or structure, and a search for the underlying cause. Let’s take them one at a time.

1. Clinical History and Examination

A thoughtful clinician starts by listening. The story of your symptoms, where they started, how they have spread, when they are worst, and what accompanies them, carries an enormous amount of diagnostic information. On examination, a doctor may test your ability to feel pinprick and temperature, since those sensations travel along small fibers. In classic SFN, these small-fiber sensations are reduced while strength, reflexes, and vibration sense (large-fiber functions) remain normal. That mismatch is a strong clue.

2. Skin Biopsy and Intraepidermal Nerve Fiber Density (IENFD)

The test most often described as the reference standard for confirming small-fiber neuropathy is a skin biopsy used to measure intraepidermal nerve fiber density, or IENFD.

This sounds more intimidating than it is. A clinician uses a small punch tool to take a tiny sample of skin, usually about three millimeters across, typically from the lower leg above the ankle, sometimes with additional samples higher up the leg or on the thigh. The sample is sent to a specialized lab where the thin nerve fibers within the skin are stained and counted under a microscope. In small-fiber neuropathy, the number of these tiny nerve endings in the skin is reduced compared with established normal values matched for age and sex.

The beauty of this test is that it looks directly at the small fibers, the very fibers that standard EMG cannot see. It is minimally invasive, generally well tolerated, and does not require stitches in most cases. If your workup has not yet included this test and small-fiber neuropathy is suspected, it is a reasonable thing to ask your neurologist about.

3. Quantitative Sensory Testing (QST)

Quantitative sensory testing measures how well you perceive specific sensations, particularly warmth, cold, and heat-pain, which are carried by small fibers. During QST, a device applies carefully controlled temperature or vibration stimuli to the skin, and you indicate when you first feel them. Because it depends on your responses, QST is considered a “psychophysical” test, meaning it reflects the whole pathway from nerve to brain to your report. It is useful as part of the picture but is generally used alongside, rather than instead of, skin biopsy.

4. Autonomic Testing

When autonomic symptoms are present, additional tests can assess how well the small autonomic fibers are working. These may include sweat testing (such as QSART, which measures the sweat response), tilt-table testing to evaluate blood pressure and heart rate responses to position change, and other cardiovascular reflex tests. These are especially useful when lightheadedness, sweating changes, or digestive symptoms are part of the story.

5. The Search for a Cause

Diagnosing SFN is only half the job. The other half, and arguably the more important half, is finding out why it is happening. This is where bloodwork and a broader medical evaluation come in, because the cause determines the treatment. We will look at causes next.

Common Causes of Small-Fiber Neuropathy

One of the reasons I feel so strongly about the education-first approach is that most small-fiber neuropathy traces back to a systemic, whole-body cause. Treating the burning feet without finding the underlying driver is like mopping the floor while the faucet is still running. Here are the categories that come up most often.

Diabetes and Prediabetes (Impaired Glucose Metabolism)

Disordered blood sugar is among the most common causes of small-fiber neuropathy, and importantly, this includes not only full diabetes but also prediabetes and impaired glucose tolerance. In fact, small-fiber damage can be one of the earliest signs of a blood-sugar problem, sometimes appearing before a person is ever diagnosed as diabetic. This is why testing for glucose problems, including a fasting glucose, a hemoglobin A1c, and sometimes an oral glucose tolerance test, is a cornerstone of the SFN workup. It is also encouraging news, because blood-sugar-related nerve stress is an area where lifestyle changes can genuinely matter.

Vitamin and Nutritional Factors

Certain nutritional issues can contribute to nerve trouble. Vitamin B12 deficiency is a classic and eminently checkable one, which is why B12 is a standard part of the bloodwork. Other B vitamins, as well as excessive intake of vitamin B6, can also play a role. Because these are simple to test and often simple to address, they should not be overlooked.

Autoimmune and Inflammatory Conditions

The immune system can attack small nerve fibers, either directly or as part of a broader autoimmune condition. Conditions associated with SFN include Sjögren’s syndrome (which classically causes dry eyes and mouth), lupus, sarcoidosis, celiac disease, and certain other inflammatory disorders. Some patients have small-fiber neuropathy driven by an immune process even without a formally named autoimmune disease. Autoimmune causes are particularly worth pursuing when the pattern is patchy or non-length-dependent, or when autonomic symptoms are prominent.

Thyroid, Kidney, and Other Metabolic Conditions

Thyroid dysfunction, chronic kidney disease, and other metabolic disturbances can all contribute to peripheral nerve problems and are typically screened for during a complete evaluation.

Infections and Medications

Certain infections, including HIV and hepatitis C, are associated with small-fiber neuropathy. Some medications, particularly certain chemotherapy agents, can be toxic to small nerves. A careful medication and health history helps surface these possibilities.

Genetic Causes

In a subset of people, particularly those with early-onset or unexplained SFN, there are inherited conditions that affect the small fibers, including certain sodium-channel gene variants and a metabolic condition called Fabry disease. These are less common but can be important to identify, sometimes for family members as well.

Idiopathic Small-Fiber Neuropathy

Despite a thorough search, a meaningful proportion of small-fiber neuropathy remains idiopathic, meaning no clear cause is found. This is understandably frustrating, but it does not mean the search was pointless. Ruling out treatable causes such as blood-sugar problems, B12 deficiency, thyroid disease, and autoimmune conditions is essential, because those are the ones where addressing the root can change the course. And research in this area continues to advance, so a cause that is elusive today may become identifiable in the future.

Management: A Two-Track Approach

When people ask me how small-fiber neuropathy is managed, I describe it as running on two tracks at the same time. The first and most important track is treating the underlying cause. The second is managing the symptoms so that you can live, sleep, and function more comfortably while the first track does its work.

Track One: Treat the Underlying Cause

This is the heart of real SFN care, and it belongs squarely in the medical domain.

  • If blood sugar is the driver, then improving glucose control through nutrition, physical activity, weight management where appropriate, and medical management can slow or in some cases stabilize the nerve stress. Early attention to prediabetes is especially valuable.
  • If a B12 deficiency is found, replacing it is straightforward and worthwhile.
  • If an autoimmune condition such as Sjögren’s is identified, working with a rheumatologist or neurologist on that condition addresses the source of the nerve attack.
  • If a thyroid, kidney, or other metabolic issue is present, treating it is part of caring for the nerves.
  • If a medication or toxin is implicated, your physician can weigh adjustments.

You can see why the workup matters so much. Each cause points to a different door.

Track Two: Manage the Symptoms

Even while the underlying cause is being addressed, or when the neuropathy is idiopathic, symptom management can meaningfully improve quality of life. This is a conversation to have with your physician, and options commonly discussed include:

  • Medications for nerve pain. Certain prescription medications developed for neuropathic pain, including some anti-seizure and antidepressant-class medicines that calm overactive nerve signaling, are frequently used. These require a physician’s guidance to select and dose appropriately.
  • Topical treatments applied to the skin for localized burning or pain.
  • Lifestyle foundations. Regular gentle exercise, good sleep habits, blood-sugar-friendly nutrition, limiting alcohol, and not smoking all support nerve health. These are not glamorous, but they are genuinely part of the picture.
  • Foot care and safety. Because sensation can be altered, careful attention to foot protection and skin checks helps prevent injuries you might not otherwise feel.
  • Support for the whole person. Chronic nerve pain wears on mood, sleep, and stress. Addressing those dimensions, sometimes with counseling, pain-psychology strategies, or support groups, is not a side issue; it is part of good care.

Where Does Chiropractic Care Fit? An Honest Answer

I want to be direct with you, because you deserve honesty more than you deserve a sales pitch.

There is essentially no peer-reviewed evidence that chiropractic care, including upper cervical chiropractic, treats or reverses small-fiber neuropathy. SFN is, in most cases, a systemic and metabolic condition. Its causes live in blood sugar, vitamin levels, the immune system, and other whole-body processes, not in the alignment of your neck. It would be misleading for me, or for any chiropractor, to suggest that adjusting your spine will fix damaged small nerve fibers in your feet. I will not tell you that, because it would not be true.

  • Rapidly progressive weakness, or any new muscle weakness at all, especially if it is spreading quickly. Pure SFN typically spares strength, so new weakness suggests something else that needs prompt assessment.
  • Prominent or worsening autonomic symptoms, such as fainting, severe lightheadedness on standing, a racing or irregular heartbeat, or significant digestive or bladder problems.
  • Sudden, severe, or rapidly spreading symptoms, rather than the slow creep that is more typical.
  • Symptoms following a non-length-dependent pattern, appearing in the face, trunk, or in scattered patches, which can point toward inflammatory or autoimmune causes worth investigating sooner.
  • Any signs of infection, foot wounds, or ulcers, particularly if you have reduced sensation and may not feel an injury.

When in doubt, reach out to your medical provider. Timely evaluation of these features can make a real difference.
