- No vestibular complaints
- Normal neurological exam findings
- Symmetric, normal-range hearing on audiometry
These patients didn’t fit the classic noise-induced or age-related profile. Their symptoms changed with neck movement, jaw clenching, or pressure on certain muscles — meaning the auditory perception was being modulated by somatosensory input from the cervical spine and craniofacial region.
Levine called this somatic (craniocervical) tinnitus and proposed that it was caused by disinhibition of the dorsal cochlear nucleus (DCN) — a brainstem region that integrates somatosensory (touch, position, and movement) input with auditory input.
