In short: “Migraine specialist” is not one kind of doctor. In Sarasota the people who treat migraine are neurologists and headache-medicine neurologists, primary care physicians, ENTs when the ear is involved, dentists for jaw-driven headache, and upper cervical chiropractors for the neck component that the other workups do not measure. This page explains who does what, how to tell which one you need first, and where Lavender Family Chiropractic fits. Call (941) 243-3729.
Why “migraine specialist” is a confusing search
If you have typed “migraine specialist Sarasota” into Google, you have probably had migraine long enough to know that your family doctor’s first two or three medications did not settle it. You want someone who deals with migraine all day. That is a reasonable thing to want, but the phrase does not map onto a single credential. There is no medical board called “migraine specialist.” What exists is a handful of provider types, each looking at migraine from a different direction, and the right first stop depends on what your migraine looks like.
Migraine is also far more common than the word “specialist” implies. The Global Burden of Disease analysis of neurological disorders published in The Lancet Neurology ranks migraine among the leading causes of disability worldwide, ahead of most conditions people think of as serious. A problem that common is handled by many kinds of clinicians, not one.
Who treats migraine in Sarasota
Neurologist. The physician who diagnoses migraine formally, using the International Classification of Headache Disorders, 3rd edition, rules out the dangerous causes of headache, and prescribes preventive and acute medication, including the newer CGRP-targeting drugs. Some Sarasota neurologists carry an additional headache-medicine certification; those are the closest thing to a “migraine specialist” in the strict sense, and they are the right call for migraine that is frequent, disabling, or changing.
Primary care physician. Where most migraine is managed, and where it should start if you have never been evaluated. Your PCP can confirm the pattern, start first-line treatment, check blood pressure and medications, and refer to neurology when the picture is complicated.
ENT or neuro-otologist. When the attacks come with vertigo, ear fullness, or hearing change, the inner ear has to be examined. Vestibular migraine is a specific diagnosis with its own criteria, and the ENT’s job is to separate it from Ménière’s disease and other inner-ear conditions. Our guide to vestibular migraine in Sarasota covers that distinction in detail.
Dentist or TMJ dentist. When the headache sits in the temples, the jaw clicks or clenches, or you wake with a sore face, the jaw joint and the muscles that drive it belong in the workup.
Upper cervical chiropractor. The provider who evaluates the top two bones of the neck, the atlas and axis, and the way they carry the head. The trigeminal nerve that drives migraine shares a processing center in the upper spinal cord with the nerves from the top of the neck, which is why neck tension, neck pain, and a prior neck injury turn up so often in migraine histories. None of the other workups on this list measures the alignment of that region. That is what we do at Lavender Family Chiropractic.
How the options compare
| Provider | What they evaluate | When they come first |
|---|---|---|
| Neurologist / headache neurologist | Diagnosis by ICHD-3 criteria, imaging when indicated, preventive and acute medication | Frequent or disabling migraine, migraine that has changed, any red-flag symptom |
| Primary care physician | Pattern, blood pressure, medication review, first-line treatment, referral | First evaluation, or migraine that responds to simple measures |
| ENT / neuro-otologist | Inner ear, hearing, vestibular testing | Migraine with vertigo, ear fullness, or hearing change |
| Dentist / TMJ dentist | Jaw joint, bite, clenching and grinding | Temple headache with jaw symptoms |
| Upper cervical chiropractor (Lavender Family Chiropractic, Sarasota) | Alignment of the atlas and axis with 3D imaging; function of the nervous system through the upper neck | Migraine with neck pain or stiffness, migraine after a neck injury or whiplash, migraine that persists despite medication, or alongside any of the above |
These are not competing camps. Most of our migraine patients in Sarasota are also under a neurologist’s care, and we send records in both directions.
How to tell which one you need first
Go to the emergency department for a headache that is the worst of your life and came on in seconds, a headache with fever and stiff neck, a headache with weakness, numbness, confusion, or trouble speaking, or a new headache after a head injury. Those are not migraine questions.
Start with your primary care physician if you have never been evaluated for migraine, or if your attacks are occasional and respond to over-the-counter medication.
See a neurologist if you have more than four migraine days a month, if preventive medication has not helped, if your pattern has changed, or if you are taking acute headache medication more than ten days a month. That last point matters: the Nature Reviews Disease Primers review of medication-overuse headache describes a secondary headache that develops from frequent acute medication use, usually on top of migraine, that is common, under-recognized, and both preventable and treatable once identified.
Add an upper cervical evaluation if any of the following is true: your migraines come with neck pain or stiffness, they started or worsened after a car accident, fall, or sports collision, they sit at the base of the skull or one side of the head, you have been through several medications without a durable change, or your neurologist has cleared you and you still want the neck looked at. Our migraine treatment in Sarasota guide explains what that evaluation involves.
What the research says about the neck and migraine
A cross-sectional study in the European Journal of Physical and Rehabilitation Medicine compared women with episodic and chronic migraine against healthy controls on upper cervical mobility, symptomatic upper cervical joints, head position sense, and posture, measuring the musculoskeletal differences at the top of the neck in migraine. A three-armed randomized controlled trial in the European Journal of Neurology followed 104 people with migraine for seventeen months, comparing chiropractic spinal manipulation against a sham procedure and usual medication, with migraine days per month tracked through twelve months of follow-up. The ICHD-3 classification is the standard every diagnosis on this page is built on, and the Global Burden of Disease analysis establishes how large the problem is. The common thread is the upper neck, which is the piece we measure and, when it is misaligned, correct. Results vary by person; we measure before and after.
What an upper cervical migraine evaluation at Lavender Family Chiropractic involves
A consultation and history that goes back to the first migraine and every neck injury since. An examination of the upper neck and the nervous system, using functional nervous system scans and paraspinal infrared thermography, neither of which involves radiation. When the case warrants it, 3D cone beam CT imaging of the atlas and axis, which shows their position in a way a brain MRI does not. If the upper neck is involved, a low-force, precisely calculated correction with no twisting, cracking, or rotation of the neck. If it is not, we say so and point you to the provider who fits. You can read each step on our what to expect page.

Common questions about finding a migraine specialist in Sarasota
Which specialist treats migraines in Sarasota? A neurologist, ideally one with headache-medicine certification, for diagnosis and medication; your primary care physician for first-line care; an ENT when vertigo or ear symptoms are part of it; a dentist for jaw-driven headache; and an upper cervical chiropractor for the neck component none of those workups measures. Many people use more than one.
Is a chiropractor a migraine specialist? No. Florida chiropractors do not hold specialty designations, and we do not describe ourselves that way. What an upper cervical chiropractor does is evaluate and correct the top of the neck, which is one of the inputs to migraine that other providers do not examine.
Do I need a referral to see you? No. We do ask for your medication list and any imaging reports, and we are glad to send our findings to your neurologist.
Should I see a neurologist before a chiropractor? If your migraine has never been formally diagnosed, or if it is frequent, disabling, or changing, yes. If you have been diagnosed and the neurologist has cleared you, the order does not matter and the two kinds of care run alongside each other.
What if my neurologist says my MRI is normal? That is usual in migraine, and it is reassuring. A brain MRI does not image the alignment of the atlas and axis, which is why a normal result does not rule the neck in or out.
How quickly can I be seen? We hold time for same-day and next-day new patient visits. See our same-day chiropractor in Sarasota page or call.
